Out Of Province/Country Tournament Request Form

Instructions:

1. Complete the registration form below (fill out separate forms for additional out of province tournament requests) at least 30 days before the tournament date.
Late requests may not receive approval.

2. Teams must purchase out of province insurance through Baseball Ontario.
